Smiles For Life, PA will be holding a dental clinic at your child’s school in a few weeks. During the clinic, a Registered Dental Hygienist will come to school and provide dental services for students who are signed up. If you have signed your child up for the clinic earlier this year your child will be seen. If you did NOT sign up your child for the clinic, (s)he will NOT be seen at this clinic. If you would like your child to participate in the spring dental clinic, please request a permission form from your school nurse or secretary.
This clinic is for people who do NOT already have a regular dentist. If your child does NOT have a dental home, and you would like her or him to have a cleaning, fluoride treatment, sealants or education about keeping his/her mouth clean and healthy, please request, fill out and return permission form as soon as possible. If your child does not have insurance or MaineCare, please include payment with your form. Thank you!
